Journey to Recovery

A goal-setting app to support stroke survivors in navigating their recovery journey and building a fulfilling life

In Australia and around the world, thousands of stroke survivors live with ongoing impairments and face a gap in support once hospital and clinic-based rehab programs conclude. This can lead to reduced motivation, isolation and stalled progress in regaining independence.

Associate Professor, Belinda Lange and her research team at Flinders University set out to develop a digital health solution that would enable  stroke survivors to continue their recovery journey beyond formal rehabilitation. The team gained the opportunity to turn this vision into reality by winning the Priority Population Digital Health Challenge 2024, which provided support to turn their idea into a fully functional solution. Cogniss enabled them to create an app that could be used both in controlled research trials to generate robust evidence, while also providing a pathway to more easily meet compliance, security and scalability requirements for broader adoption if proven effective.

Journey to Recovery, the outcome from the 12-month structured program of the challenge, is a mobile app that empowers stroke survivors to navigate life after formal rehabilitation ends by helping them set and track personal recovery goals. It is co-designed with stroke survivors and built on the Cogniss no-code platform, ensuring that it directly addresses the unique needs of people recovering from stroke in an accessible, personalized way.

Key features of Journey to Recovery

  • Personalised goal setting to define and track meaningful recovery goals

  • SMART action planning that breaks down broad goals into achievable steps with regular check-ins

  • Initial self-assessment to identify strengths and provide tailored goal suggestions

  • Accessibility features co-designed with stroke survivors, including large text, simple navigation and supportive language

  • Positive reinforcement and wellbeing tips to encourage community re-engagement and confidence

“ Cogniss is a really flexible platform that allows for a whole range of different app features to be developed,” said Belinda, reflecting on her experience. “We were able to quickly develop a prototype of our app, make changes, and test those within the app, which reduced development time and gave us the opportunity to make real-time updates throughout co-design activities.”

Journey to Recovery has launched on both Google Play and Apple’s App Store, entering its evaluation trials to measure its impact on stroke survivors’ outcomes. The app demonstrates how a great idea, backed by the right tools, can quickly become a real-world digital health solution, improving quality of life and long-term recovery for survivors in a way traditional rehab alone often cannot.
